Excluded Produce Farm Form
Use this form to determine whether your produce farm is excluded from the requirements of the FSMA Produce Safety Rule (21 CFR 112.4). Produce farms are encouraged to self-certify their excluded status at least once every five years or whenever their farm status changes.
