New Farm Request Form

*Please complete a seperate entry of this form for each facility (farmstead) of the farm operation*


The purpose of this form is to add new facilities to the Partner Database. If a change is required to an existing facility, email judson.peck@vermont.gov the WQID and the desired change instead of filling out this form.

NOTE: The information submitted in this form will be sent to the Vermont Agency of Agriculture, Food & Markets (VAAFM) Water Quality Division. Submission of the following  information is voluntary, but required to add a farm to the Partner Database. Submitting this information will not result in a farm inspection.

Entries for Physical Address should match the address for the facility as found in E911 Viewer (click for link). You can also look up E911 addresses in the Partner Database. 

NOTE: CLICK HERE to ensure that farm size is being accurately recorded. Some guidelines:

  • LFO: 700+ cows *permitted with VAAFM*
  • MFO: 200+ dairy cows (OR 300+ heifers/beef cattle) *permitted with VAAFM*
  • CSFO: 50+ cows (OR 75+ heifers/cattle, 50+ acres annual crops, 750+ sheep/goats) *certified with VAAFM*
  • SFO: $2000+ in annual sales, 4.0+ acres used for agricultural activities, OR files 1040(F) *no permit/certification required, but regulated by RAPs*
  • WQ: falls below criteria to qualify as an SFO *regulated by municipality, not the RAPs*
