New Farm Request Form

*Please complete a seperate entry of this form for each facility (farmstead) of the farm operation*

The purpose of this form is to add new facilities to the Partner Database. If a change is required to an existing facility, email the WQID and the desired change instead of filling out this form.

NOTE: The information submitted in this form will be sent to the Vermont Agency of Agriculture, Food & Markets (VAAFM) Water Quality Division. Submission of the following  information is voluntary, but required to add a farm to the Partner Database. Submitting this information will not result in a farm inspection.
